I

No apparent hair loss or recession commonly found in young adults.

II

The temple area is minimally recessed for several centimeters. An early thinning of the crown area might also be apparent.

III

The recession in the temple area is more sometimes extending to the mid-ear level. The thinning in the vertex region is more prominent compared to Type 2 but is again not usually discernible.

III (Vertex)

Same as above but the vertex is more thinned which makes one observe a bald patch over there.

IV

The hair line has recessed several centimeters beyond the mid-ear and the vertex patch has increased.

V

There is only a thin strip of hair between the recessing anterior hair line and the growing bald patch of the vertex area. The center of the vertex has lost its hair completely.

VI

The frontal region has nearly no hairs and is meeting the vertex baldness. The vertex bald region is considerably increasing.

VII

Only a limited number of hairs is present in the back of the head the rest of the scalp being totally hairless and shiny.

·         Diffuse thinning

Akin to female pattern baldness, some male patients might have diffuse thinning of hair over the entire scalp and they cannot be classified according to the Norwood scale.

·         Patchy Hair Loss

This is another category that is not covered by the Norwood scale. The entire hair loss is patchy with areas of relative baldness interspersed with areas of higher density.

   Modern Techniques of Hair Transplantation

 Basically, there are 2 modern methods of hair transplantation: FUT and FUE.

 

      FUT or Strip Technique (Follicula Unit Transplant)

 In the FUT or strip technique, a strip of hair bearing skin from the back of the scalp is taken and the area closed off with sutures. This results in a linear scar over the back of the scalp which can be minimized with trichophytic closure techniques. This is the most commonly performed surgery worldwide and is popular because a large number of follicles can be obtained and transplanted in a single session depending on the scalp laxity. Megasessions involving the transplantation of 3000-5000 grafts can be transplanted in a single session by this technique. In addition, the longevity of follicles obtained by the FUT technique is likely to be more because of the preponderance of stem cells at the base of the follicles.

 

       FUE  (Folicular Unit Extraction)

 FUE involves the extraction of grafts one by one followed by transplantation into the bald area. Contrary to popular belief, FUE is not a scar less procedure and multiple pin point scars can be seen on close examination. This method can transplant maximum 1500-2000 grafts in a single session even in the most experienced hands. Follicle loss can be considerable during harvest depending on the experience of the surgeon as this procedure is essentially blind. However, this is the sole method for extraction of body hairs. The follicles are frequently denuded and devoid of stem cells as compared to follicles extracted by the FUT technique so the longevity of these hairs is likely to be less.

 

      Combined FUT with FUE

 To increase the number of grafts harvested some surgeons initially take a strip and then harvest additional follicles as part of a megasession which gives superior results than either method alone. The advantage is that larger areas of the scalp can be covered with a higher density of follicles.

 

 

Grafts vs Follicles

It is important that you understand the difference between grafts and follicles because many hair transplant surgeons would inform you about the rate per graft or follicle.

Hairs are arranged in clusters in the scalp which might vary from 1-4 hairs. When multiple follicles are clustered close together it might be impossible to separate them without damaging a follicle. Therefore, it is implied that a single session would not achieve implantation of such a huge number of grafts. Therefore, most if not all hair transplant patients require at least 2-3 sessions for optimal coverage of the bald area. Availability of grafts is also taken into account. Patients who have lesser donor grafts in the scalp might require body hair transplantation which might increase the number of sessions because body hair harvesting can be a slow process.


 Technique of Hair Transplant
